Practice rules

In addition to the General Terms and Conditions, the following practice rules apply in our practice:

Personal data
As a new patient, you must present a valid ID and your insurance card during the first appointment. Your medical file is now linked to your citizen service number. That is why registration is only possible with a valid BSN number. Do you have a secret residential address or do you live abroad? In order to be able to register you, we need a recent letter from a recognized sender for verification (tax authorities, municipality, bank, etc.). You must notify us as soon as possible of any changes to your personal information, such as address, telephone, e-mail, etc. During your first visit, we ask you to complete a questionnaire related to your health. You must fill in this list completely and truthfully. This is to avoid possible complications. You must always notify us of any changes in your health and medication use.
During the intake appointment:
Get to know your regular attending dentist Your wishes and needs are mapped out Is the general health of your teeth visualized (by means of a clinical and X-ray examination) Your treatment plan will be discussed with you & nbsp; Please note: costs are associated with an intake appointment, just as with any other treatment.
First appointment (intake)
As soon as you have registered, we will contact you for an “intake”. In order to be able to offer high quality oral care, it is necessary for us to first get a good picture of your general oral health and your own wishes.
Reschedule or cancel an appointment

Rescheduling or canceling an appointment must be done in time. In case of a missed appointment, the treatment time reserved for you will be charged. We charge € 10 per 5 minutes reserved for treatment.

Appointments can be rescheduled / canceled free of charge at least 48 hours in advance (excluding Sundays and public holidays).

Appointments and treatments
At Dental Luxe, the focus is on keeping your own teeth for as long as possible. We therefore work preventively and prefer to prevent ailments rather than cure them. A six-monthly inspection visit is necessary to ensure this. That is why, after your six-monthly check-up appointment, we immediately schedule the next check-up appointment. The check-up appointments are generally in combination with a visit for dental cleaning with our dental hygienist or prevention employee. In principle, there is no time for follow-up treatment during the six-monthly check-up. A follow-up appointment will therefore be made if necessary. We have reserved fixed times in our agenda for the treatment of pain complaints. Unfortunately, we cannot always take into account your preferences regarding work or school hours. If you arrive late for an appointment, you run the risk that the treatment cannot be completed as planned or even canceled due to lack of time.
Appointment reminder by SMS or E-mail
A few days before your appointment, you will receive a reminder by SMS or e-mail (you can indicate your preference at the desk). However, this is intended as an extra service and no rights are derived from it. Keeping track of appointment times remains your responsibility.
Also important:
Please report changes in address, insurance, telephone and medical data (in good time) at the counter or by telephone. If there are any changes in your medication or if you are pregnant, do not forget to inform your dentist. Some treatments can pose a health risk. In practice, pets are not allowed. Children up to the age of 10 must be accompanied by a parent or adult accompanying person at every practical visit. Watch over your belongings yourself. Dental practice Dental Luxe is not liable for damage, loss or theft of your belongings. We therefore advise you not to leave your belongings unattended. Within our practice, various activities are delegated to trained and authorized employees. You can indicate your preference for a practitioner and you will always keep your regular practitioner. However, in the unforeseen absence of your preferred practitioner (s), it may happen that your appointment is taken over by a colleague employee. If possible, we try to inform you in advance. We always strive to keep the waiting time as short as possible. However, it may happen that you have to wait longer because, for example, some patients require more time than expected or because urgent patients come in between. We do our best to keep you informed should this occur. We ask for your understanding for this.
